Job Title: Medical Records Clerk

Summary of Position Description: Position is responsible for maintaining patient medical records by locating, distributing, filing documents and re-filing charts.

Primary Duties Include:
  • Printing appointment schedules and pulling charts daily two days in advance
  • Pulling charts and new patients messages and delivering to the triage department
  • Scanning all labs x-ray reports including documentation/correspondence in chart in a timely manner
  • Combining all ultrasound images to one PDF and attach to orders in patient chart
  • Re-filing all charts daily and maintain chart walls
  • Processing in-coming and out-going mail /distribute faxes
  • Preparing and copying records for Release of Information requests
  • Purging walls of the outdated charts, as directed
  • Preparing and keep log of charts sent offsite to be scanned into Electronic Medical Record
  • Preparing charts for optical scanning (archiving)
  • Pulling charts for yearly chart audits
  • All other duties as assigned
Job Qualifications:
  • High School Diploma or equivalent
  • One year office work experience preferred
Physical Requirements:
  • Hearing, speaking and visual acuity
  • Must have the ability to lift at least 25 pounds
  • Ability to walk, bend, stoop, reach overhead, stand for long periods of time
  • Hand/wrist movement to write, use computer, phone and copier
